Within the Assessment and Intervention team, Social Workers, Physiotherapists, Occupational Therapists and Speech
Language Pathologists work with individuals having a disability and their
families/guardians, to provide support
in relation to their presenting concerns in a holistic and effective way. Through a multidisciplinary
approach, the team aims to reach the desired objectives of the service user and
his/her family/carer through assessment, short-term and/or long-term
interventions.
The team seeks to promote the
recognition, integration and social inclusion of the person with
disability as a full member of society and the communities that they from part
of. In adherence with the Agency's holistic and person-centred approach, the
Assessment and Intervention Team provides individualised support services that
enable and empower persons with disability to recognise their abilities, realise
their vision and enhance their quality of life. The team strives to advocate
for and uphold community inclusion, as this provides a diverse community. This
also ensures that everybody has the same opportunities to participate in every
aspect of life to the best of their abilities and desires.
Intake: Responds to service requests and proceeds with follow-up action
plans incorporating different professionals;
Coordination: Supports
persons having a disability and their families in all decision-making processes
that affect them, and coordinates formal and informal service provision;
Advocacy: Provides
advocacy services along with, or on behalf of, a person with disability whose
rights to services or opportunities may actually or potentially be compromised.
This service includes assisting such persons to participate in all aspects of
community life (education, employment, recreation, home, social, and more);
Screening and
Assessment: Provides screening and assessment in the areas of community
involvement, social inclusion, emotional wellbeing, education and employment,
mobility and accessibility, communication difficulties, learning difficulties
and other therapeutic interventions;
Interventions: Plan and delivery intervention programs based
on the individualised needs of the referred person with disability.

Social Work Intervention
· Occupational Therapy Intervention involves the process and
skilled actions taken by occupational therapists in collaboration with the
client, family/guardians and multidisciplinary team to facilitate engagement in
occupation related to health and participation. The intervention process
includes the plan, implementation, and review.
· Physiotherapy Intervention involves the use of physical
approaches towards the promotion of health, and the prevention,
treatment and management of disease and disability, focusing on the mobility
and physical concerns of the client.

Speech and Language Pathology Intervention
Review:
Conducts and/or participates in regular reviews to ensure effective service
provision in relation to Agency service users.
Apart from interventions and assessments within the community, the Assessment and Intervention Team within Aġenzija Sapport complements the other services offered by the Agency through the required professional services.
