Blue Badge Documents

New application for a Blue Badge:
  • Application Form and Medical Certificate. Please make sure that the applicant / guardian signs in the space provided. Medical Certificate needs to be signed and stamped by a Doctor. 
  • One (1) recent, coloured passport photo (40mm X 30mm) of the person with disability certified on the back. 
  • €11.50 administration fee. Cash payment will not be accepted by post. Exact amount is to be presented. 
  • If one is applying on behalf of the applicant, one must also provide a copy of their ID Card.  

Forms are to be submitted by hand to Aġenzija Sapport, Patri Ġwann Azzopardi Street, Santa Venera.
Office opening hours: Monday to Friday from 08:00 - 15:30​


Renewal of a Blue Badge:
  • Application Form and Medical Certificate
  • One (1) recent, coloured passport photo (40mm X 30mm) of the person with disability certified on the back.
  • If one is applying on behalf of the applicant, one must also provide a copy of their ID Card.  

No fee applies for application of renewal of the Blue Badge.

Application forms for renewal of Blue Badge may be submitted by email on bluebadge.sapport@gov.mt​ and attach all the necessary documents or by handing in the application at Aġenzija Sapport, Patri Ġwann Azzopardi Street, Santa Venera.
