Referrals and Assessments

Persons with disability of up to 59 years having any type of impairment may benefit from Social Work Services. Persons with disability who are 60 years of age and over are directed to the Social Work Services for the Elderly. The Service follows the below criteria:
  •  priority is given to individuals at high risk of institutionalisation, or in high need for support;
  •  requests are assessed immediately by the intake Social Worker, who will provide as much support as possible during and soon after the intake. If the service user and his/her family require long-term support, the case is put on a waiting list (depending on the urgency of the support required). Length of time on the waiting list is not a factor in deciding when one will benefit from Social Work Services;
  •  the service is voluntary which means that, the person with disability, his/her family, and/or service providers, request the service out of their own accord and are willing to participate in the social work process.
Upon receiving a request, the Social Work Services will carry out the intake assessment and arrange for follow-up services. Should a request be made for either Community or Residential Services, the team of Social Workers will short-list the referrals based on the significant and critical needs of the individuals and submit their assessments to the Service Allocation Committee (SAC) for allocation of services.
