Youth work is dedicated to supporting the holistic development of young people. When applied to persons with disabilities, it takes on a particularly empowering role – focusing on personalised, community-based support that nurtures each young person’s potential.

 

The Youth Work Service at Aġenzija Sapport provides young persons with disabilities a safe, inclusive, and empowering environment where they can thrive.

 

The aim of this service is to:

  • promote the full inclusion of disabled youth in society
  • enabling them to exercise their rights
  • build meaningful relationships
  • actively participate in community life

 

By means of individualised support plans, structured one-to-one sessions, and creative group activities, the service promotes:

  • informal education
  • active citizenship
  • personal and social development

 

Through a strengths-based and rights-focused approach, Youth Work fosters independence, self-confidence, and social engagement, recognising young persons with disabilities as valued and capable members of society.

 

The service’s team  collaborates with other professionals, ensuring holistic and person-centred support. Progress is monitored regularly to adapt to each young person’s evolving needs and goals.

The service aims to foster autonomy, inclusion, and meaningful participation in the community.

 

Eligibility

  • Young persons with disabilities, between the ages of 12 and 35 years.
  • Individuals who are already service users of Aġenzija Sapport or referred through the Intake Team following an initial assessment.
  • Participation is voluntary, meaning the young person must be willing to engage in the service.
  • The service is available for both group activities and one-to-one sessions, depending on the individual’s needs and preferences.

Referrals and Assessments

Referral requests can be made by professionals, family members, or the young persons themselves, and must include relevant details such as diagnosis, goals, and availability.

 

The intake team receives and processes all intake requests for any service within the Agency. All intakes are meticulously assessed by the Intake professionals, who will identify which service/s the individual can benefit from dependent on the needs, interests and wishes of the individual.

 

Following this assessment, the team will liaise accordingly with the professionals from the departments of the services identified and carry out follow-up action plans.

 

If already supported by Aġenzija Sapport, service users may reach out to their assigned professional, who will initiate the referral process.

 

Upon receiving the referral, the Youth Work Team will schedule a meeting with the referring professional to discuss the best way forward.

 

The Youth Work Team will make direct contact with the young person to begin the support process.
