The Intake Team is the first point of contact for any person seeking to request and initiate services within Aġenzija Sapport. Intake is the initial process of service delivery within Aġenzija Sapport. The service’s role is to support prospective service users and their family/ caregivers in identifying needs, define goals that they desire to acquire, and consider the different internal and external services provided, which may be appropriate to help meet such needs and achieve the mentioned goals.

Eligibility
During the initial contact, the team ensures that the prospective service user meets the Agency services’ eligibility criteria, that is:
- a disability diagnosis is present;
- the service user’s age is between 0 to 60 years;
- the service user consents to receiving support.
In line with the Agency’s values, the Intake Team assures that the service user’s self-determination is practised from the initial phases of service delivery by attaining their views on needs and goals and including them in the formulation of plan of action. This is done through contact established between the Intake professional and service user, family, and other professionals involved. Such contact includes face-to-face visits, such as home visits and other interventions deemed necessary according to the service user’s situation. Through such visits, the Intake professional can gather necessary information to support the service user who can then have the opportunity to better understand any services available at their disposal.
Referrals
The Intake Team receives and processes all requests by prospective service users, families, and other professionals who ask for any service within the Agency. All intakes are meticulously assessed by the Intake professionals, who will identify which service/s the individual can benefit from depending on the needs, interests and wishes of the individual.
Ultimately, the intake process is concluded by formulating a plan between the Intake professional and prospective service user on how to access the desired services to meet the agreed goals. The professionals then work on the necessary referrals and link the service user to other services, both internal and external services to support with their needs effectively.
